The following form, which is expressly designed to be completed locally on your computer using your Acrobat Reader Plug-in or Acrobat Reader Application Program, offers simplicity and privacy. Just tab to the fields and enter the data required. When finished, all you have to do is print the form, sign it, and fax it back. It is not saved nor sent anywhere on/from our web site. Your signature confirms that your order is intentional. Because this form is completed locally on your computer no data is exchanged or captured by a server. The only response we receive is the fax you send. It is received by our fax machine and is only handled by our accounting personnel.
